When you’re attempting to conceive, it’s natural to monitor your menstrual cycle more closely. After all, pregnancy requires ovulation to occur.

Many people assume that having a monthly bleed indicates normal ovulation. Surprisingly, that isn’t always true.

Ideally, a woman’s reproductive system will ovulate each month. However, there are times when ovulation doesn’t happen — a situation known as anovulation. When that occurs, you may still notice bleeding and think it’s your period. But if the cycle was anovulatory, that bleeding isn’t technically a true menstrual period.

What’s an anovulatory cycle?

As the term implies, an anovulatory cycle is a menstrual cycle in which a woman does not ovulate. During ovulation, an ovary releases an egg (oocyte).

It’s not unusual for women in their prime fertility years to have an anovulatory cycle now and then. You might have experienced one without realizing it. That’s because anovulation can still be accompanied by bleeding that resembles a normal period.

In a typical cycle, progesterone production is prompted by the release of an egg. This hormone supports regular menstrual bleeding. In an anovulatory cycle, low progesterone may result in heavier bleeding, which can be mistaken for a regular period.

Bleeding in anovulatory cycles can also be due to an overgrowth of the uterine lining, the endometrium, which becomes unsustainable and sheds. A decline in estrogen can similarly trigger bleeding.

Why do women experience an anovulatory cycle?

Anovulatory cycles are most commonly seen in two age groups:

  • Adolescents who have recently started menstruating: During the first year after menarche, girls are more prone to cycles without ovulation.
  • Perimenopausal women: Women in their 40s to early 50s often experience hormonal shifts that increase the likelihood of anovulatory cycles.

Both age groups undergo substantial physiological changes. Rapid swings in hormone levels may provoke anovulation. Other contributing factors include:

  • excessively low or high body weight
  • intense or excessive exercise
  • disordered eating or poor nutrition
  • significant stress

If your periods occur every 24 to 35 days, you’re likely ovulating regularly.

In the United States, 10 to 18 percent of couples face difficulty conceiving or maintaining a pregnancy. Persistent anovulation is a frequent cause of infertility.

How is anovulation diagnosed?

Diagnosing anovulatory cycles can be straightforward when a woman has absent periods or very irregular bleeding. But for others, diagnosis can be more challenging.

To evaluate for anovulation, your clinician may assess:

  • serum progesterone levels
  • the thickness and appearance of the uterine lining
  • blood tests for specific antibodies or hormonal markers

An ultrasound exam may also be performed to visualize the uterus and ovaries more closely.

Treatment for anovulation

Results from these tests guide the choice of treatment.

If the anovulatory cycles are linked to modifiable factors like diet or lifestyle, addressing these areas is often effective — improving nutrition, adjusting exercise patterns, and achieving a healthy weight (gain or loss as recommended by your provider) can restore ovulation.

When internal hormonal imbalances are the cause, your doctor may offer fertility medications.

Medications target the underlying issue — they can stimulate follicle maturation, boost estrogen levels, and encourage the ovaries to release an egg.

If a significant structural problem such as a tumor is identified, surgery may be necessary.
